This study aims to determine whether the use of aciclovir, a highly effective antiviral drug, can improve the outcome of patients with genital herpes in an HIV-1 infection. A total of 12 HIV-1 patients were enrolled from the CDC, and their treatment was initiated at the Department of Medicine and Infectious Diseases, University Hospital, Berlin (Germany). All patients were randomly divided into 4 groups: (1) the control group, who received no treatment; (2) the aciclovir group, who received aciclovir and received either 800 mg of aciclovir daily or 800 mg of aciclovir as a suppository; (3) the aciclovir group receiving aciclovir for 5 days; (4) the aciclovir group receiving aciclovir for 5 days and the aciclovir group receiving aciclovir for 5 days, respectively. All patients received a standard of care (oral suspension of 5 mL) for HIV-1 infection. A single-dose oral aciclovir was administered to each patient. The patients were assessed for genital herpes, and the outcome was assessed using genital ultrasound. Results showed that the aciclovir group had a significantly higher incidence of genital herpes compared to the control group (30.7 vs. 12.3%, respectively, p<0.001), but the aciclovir group did not experience any significant difference between the aciclovir group and the control group (p=0.26).
